Personal Wellness Plan and Wellness Resources on Campus Writing Paper

*Paper has 3 parts. Each part is worth 25 points. The final 25 points is earned by writing a college level paper, in paragraph format with proper grammar and sentence structure. Writing in bullet points will result in a 0/25 points for the fourth section.

I. Personal Wellness Plan

A.) Readiness to change (Pre-contemplation, Contemplation, Maintenance, etc.)

– We went over 8 dimensions of wellness throughout the semester. In the introduction chapter, we discussed the Readiness to Change Model. For EACH of the 8 dimensions, discuss which stage of this model you are in.

B.) Pick 3 dimensions where you are ready to make a change in your life! These are goals you want to focus on and are ready to improve.

– For each goal, discuss your short-term goal (4-6 weeks) and your long-term goal (3+ months). Also discuss your plan to reach these short-term and long-term goals in DETAIL.

C.) This portion of the paper should be 3-4 pages.

II.) Health Appraisal Day (HAD)

  • Did you participate in HAD?
    • Utilize your orange sheet and discuss areas that are your strengths and weaknesses.
    • You must touch on BOTH strengths and weaknesses!
  • Did you miss HAD?
    • Do basic demographic stats for your body (height, weight, BMI, blood pressure, etc.)
    • Follow the same requirement as above
  • This portion of the paper should be 1-2 pages.

III.) Wellness Resources on Campus

  • For each of your 3 detailed dimensions from part 1, what resources are available to you on campus that can help you reach your goal?List the location and the hours of the resource and how they can help you.
  • This portion of the paper should be 1 page.
